White Plains Hospital Receives 2025 ANCC Magnet Prize® Sponsored by Press Ganey
New York hospital recognized for pioneering nurse-led remote patient monitoring model that improves outcomes for patients with diabetes
ATLANTA –– October 8, 2025 –– The American Nurses Credentialing Center (ANCC) announced today that White Plains Hospital in New York received the 2025 ANCC Magnet Prize®, sponsored by Press Ganey. The recognition was announced during the 2025 ANCC Magnet & Pathway Conference™ in Atlanta.
The ANCC Magnet Prize is presented annually to a Magnet® designated organization whose nursing team best demonstrates exemplary innovation in patient care delivery, including notable achievements within both practice and research, resulting in improved patient health outcomes. White Plains Hospital will use the $125,000 award to further develop its project, Revolutionizing Diabetes Transitional Care: A Nurse-Led Remote Patient Monitoring (RPM) Model to Advance Equity and Outcomes.
This innovation addresses fragmented post-discharge care and inadequate glycemic oversight experienced by people with poorly managed diabetes. When left unchecked, these oversights can lead to preventable complications which disproportionately affect people in marginalized communities.
The RPM Model is a 90-day transitional care model that incorporates continuous glucose monitoring, structured clinical coaching, and a pathway to improved outcomes for patients. Nurses access the continuous glucose monitoring data through a HIPAA-compliant dashboard to help inform structured education, behavioral coaching, medication review, and psychosocial support to help stabilize glycemic patterns and avoid complications.
Health equity is a hallmark of this program as about 61% of patients undergoing treatment via the RPM Model are either ethnic minorities or uninsured. The program has been tailored to proactively address food insecurity, transportation, and medication access while mitigating social barriers to chronic disease management. The nursing team at White Plains Hospital plans to use this funding to expand accessibility to more vulnerable patients, initiate robust research on this topic, and build a standardized dissemination toolkit.
“We are honored to accept this award on behalf of all our extraordinary nurses who deliver exceptional patient care to our community every day,” said White Plains Hospital Senior Vice President of Patient Care Services and Chief Nursing Officer Kerri Elsabrout, DNP, MBA, RN, FNP-BC, NEA-BC . “At White Plains Hospital, we challenge our nurses to bring a critical perspective to caregiving and research – to always challenge the status quo in order to provide the best care. We are so proud that our commitment to community-based care allowed for the design of an innovative model of care delivery that will make a difference for countless patients with diabetes.”
“Congratulations to our partners White Plains Hospital on receiving the 2025 ANCC Magnet Prize®,” said Jeff Doucette, DNP, RN, NEA-BC, FACHE, FAAN, senior vice president and chief nursing officer, Press Ganey. “This nurse-led model for transitional diabetes care is a powerful example of how innovation and equity can transform patient outcomes. By leveraging real-time monitoring and proactive coaching, the team is reimagining chronic disease management and proving the vital role nurses play in leading technology-enabled care. We are honored to sponsor this recognition and look forward to seeing how this program inspires broader adoption across health systems nationwide.”
“The nurses at White Plains Hospital are unwaveringly committed to wholistically improving the health of their patients, meeting them wherever they are to help them overcome various barriers to receiving quality healthcare,” said American Nurses Enterprise Chief Nursing Officer Brad Goettl, DNP, DHA, RN, FAAN, FACHE. . “White Plains Hospital expertly demonstrates the wide-ranging benefits of achieving Magnet status, advancing nursing excellence by valuing their nurses and their contributions, and setting the standard for quality care.
